Instructions

After attending a meeting or conference for your assigned organization, please complete the following report form within 10 days. Member expense forms cannot be processed without a completed report.

When filling out the form, focus on providing information that is relevant to CTA. If the report includes pending legislation, please refer to bills by number and author (AB/SB XXXX). If the report includes reference to specific agenda items, please include those item numbers.

A recommendation is: A specific request for CTA to take action (i.e., write a letter, arrange for a CTA speaker, sponsor event(s)*, or send member(s) to conference). Please be specific and give rationale. *CTA liaisons are not to attend a meeting or conference at a "Unite Here's Do Not Patronize" venue.
